How can I get a high score in TOEFL Speaking test?

I'm doing my TOEFL test in 25 days. I'm very nervous, I took a course with a local teacher about a month ago, but the Speaking section is very difficult for me. It needs multiple speaking skills like talk on a topic, giving an introduction with 2 main reasons and supporting reason with 2 examples and then making a conclusion, all this in only 40 seconds, and there is only 15 seconds to plan your answer.
